Help shape Youth Focus
Youth Focus exists to create a world where young people’s mental health doesn’t get in the way of who they want to be. As a young person, you are the expert in how we can do that. We would love to hear how you find our service, and what we should do differently when supporting young people.
We are running this short survey for two weeks to hear from you on how we’re doing, and what we should do differently.
The survey was co-designed with young people from our Youth Reference Groups in Perth and Albany.
Your anonymous responses may appear on our website or social media. However, your name or personal details will never be included. Your answers will never impact your ability to get support from us or any other service.
You can ask a clinician or support person to help you fill out the survey, if you want.
At the end of the survey, you can enter a draw to win one of four $50 vouchers. The contact details you provide will only be used to send the voucher if you win.
The survey should take about 5 minutes to complete. Please talk to your Youth Counsellor for the link.

FAQ
We will share the (anonymous) results with our teams and our Youth Reference Groups, to decide what specific changes we can make to improve our services, using your feedback. We also might use some statistics and quotes on our website, reports and social media.
There are four $50 vouchers available to be won, across all of our services. Once the survey closes, we will use a random number generator to pick 4 winners, from the young people who have opted in to the draw. We will then email or text the voucher to you (if you win!). You will get your voucher by Thursday 18th December at the latest.
Yes. Even if you decide to enter the draw, your contact information will be separated from the rest of their responses, and then deleted after the draw. Your individual response will not be connected to you or used as part of your support.
You can talk to the team member that supports you about the survey, if you like. Or feel free to contact Ellie in our Service Excellence team with specific questions or feedback.
